UGPC CLUB OUTING WAIVER AND MEMBERS RESPONSIBILITY

Your acceptance of this application indicates your acceptance of these terms

I have been given permission by the Utah Gold Prospecting Club to accompany them on recreational excursions. I agree that I am participating in these excursions of my own free choice and that I am not giving the Utah Gold Prospecting Club any consideration order to participate. I, with full knowledge of the consequences, hereby waive any claim for injury, theft, loss of life, or any other damages whatsoever to me or to a member(s) of my family, except that which may result from willful misconduct. In the event any claim made against the Utah Gold Prospecting Club by me, resulting from my participation on the excursions. I agree to hold the Utah Gold Prospecting Club harmless and indemnify it and its members for all costs and expenses, including legal fees incurred by the Utah Gold Prospecting Club and its members.

I, being a member and representative of the Utah Gold Prospecting Club, will abide by its by-laws and be of good standing at all times. I will show professional courtesy to each officer and members at all times. Any disputes or personal disagreements and or conflicts that can’t be resolved in a respectful manner, should be addressed thru a mediator for resolution. (PRESIDENT AND VICE PRESIDENT ONLY) The President and Vice President reserve the right to initiate immediate resolution to diffuse any inappropriate behavior. (I.E. fighting both verbal and or physical, excessive offensive language, drunkenness and or any slanderous acts.) These actions could result in immediate disciplinary action and or removal of one’s membership permanently from the Utah Gold Prospecting Club, if this is a reoccurring problem, unless otherwise resolvable.
